By entering into this Standardized Data Sharing Agreement, I, hereafter “Collaborator”, agree to provide coded* data and accompanying metadata to the PAIN Standardized Repository at the Oppenheimer Center for Neurobiology of Stress at the University of California, Los Angeles, hereafter “Recipient”, and to adhere to the Data Sharing Policies as established by the Executive Committee of the Pain and Interception Imaging Network (PAIN) Repository.

*Data are separated from personal identifiers through the use of a code. The code key will be held by the contributor only, and the PAIN Repository will never retain this linking file or have access to the code. PAIN data do not contain any personal identifiers.

Terms used in this agreement but not otherwise defined, will have the meaning given by the Office for Human Research Protections regulations.

By becoming a member of the PAIN Standardized Repository, Collaborator agrees to provide chronic pain neuroimaging data following the PAIN Standardized Repository protocol described on the MRI Imaging Acquisition Manual, and metadata from at least 20 subjects per year. Except as otherwise specified herein, Collaborator authorizes Recipient to share and utilize the coded data for large scale, multi-center studies that will combine data from similar populations.

Collaborator is responsible for providing a copy of the existent study IRB approval or equivalent, the study protocol and consent forms to the Center for Neurobiology of Stress prior to the data submission to the PAIN Standardized Repository.

This Data Sharing Agreement applies to both the raw data received from a principal investigator as well as any copies made by the Recipient research team, and any new data derived solely or in part from the data.

Except as otherwise specified herein, Recipient may make all uses and disclosures of the coded data set described above necessary to maintain the PAIN Standardized Repository. Data provided by the Collaborator may be destroyed and removed from the PAIN Standardized Repository upon the written request of the Collaborator.

The terms of this PAIN Standardized Data Sharing Agreement shall be effective as of today’s date and remain in effect until membership to the PAIN Repository is cancelled by written request by either party.

Collaborator agrees that failure to abide by the guidelines included in this Standardized Data Sharing Agreement and/or on the PAIN Repository Data Sharing Policies will result in termination of membership to the PAIN Repository and privileges in accessing PAIN Repository data.

Both Collaborator and Recipient agree that it will only be responsible for its own acts and the results thereof, and not responsible for the acts of the other party/parties and the results thereof.

Terms used in this agreement but not otherwise defined, will have the meaning given by the Office for Human Research Protections regulations.

By becoming a member of the PAIN Archived Repository, Collaborator agrees to provide neuroimaging data from chronic pain subjects and/or healthy controls, study methods (if available), and a minimum set of metadata to the PAIN Archived Repository. Except as otherwise specified herein, Collaborator authorizes Recipient to share and utilize the coded data for large scale, multi-center studies that will combine data from similar populations.

Collaborator is responsible for providing a copy of the existent study IRB approval or equivalent, the study protocol and consent forms to the Center for Neurobiology of Stress prior to the data submission to the PAIN Repository.
